Mercy Gives $576,374 in Grants to Help Thousands of People in Need
31 local non-profits receive grants to help fund programs
in Sacramento, Nevada, Placer and Yolo counties

SACRAMENTO, January 7, 2010 - Catholic Healthcare West (CHW), which operates the local Mercy hospitals, has awarded $576,374 in grant funding to 31 non-profit organizations in Nevada, Sacramento, Placer and Yolo counties.
 
The grants, from $5,000 to $25,000, will impact health issues ranging from mental illness to chronic disease management, and community-based programs that provide services from birth to end-of-life. This year's recipient organizations reach out to local ethnic communities and provide services to multiple generations from infants to the elderly.
 
"Every year we are able to help deliver critical healthcare and social services to people who need it most through our Community Grants Program," said CHW's Sacramento Service Area President Tim Moran. "We are proud to work collaboratively with so many outstanding  nonprofit organizations to improve the health and well-being of the communities we serve."
 
"Last year in the greater Sacramento region, CHW and the Mercy hospitals distributed $526,000 to deserving projects and organizations," said Moran. "Despite the economic crisis, this year is no different. And with state and county budget cuts, the need is greater. This year, we are awarding more than $576,000 to support local non-profit efforts."
 
The CHW Community Grants Program supports community-based organizations that provide services to individuals, families and children in need. The grants program is open to all non-profit service organizations with an interest in building healthier communities by improving health and living conditions. The program supports organizations that increase access to healthcare and address the underlying causes of illness - such as physical and substance abuse, neglect, poverty and homelessness - as well as specific health issues. The grants are funded by contributions from Catholic Healthcare West's member hospitals. In the Sacramento region, CHW hospitals that contribute to the grants program are Mercy General Hospital, Methodist Hospital of Sacramento, Mercy San Juan Medical Center, Mercy Hospital of Folsom, Woodland Healthcare and Sierra Nevada Memorial Hospital.
 
This year's Community Grants were awarded to the following Sacramento-area organizations:
  • American Cancer Society - provides transportation assistance to underserved cancer patients, specifically the elderly.
  • Bishop Gallegos Maternity Home - Provides shelter and prenatal care for pregnant mothers or mothers with infants.
  • Catholic Social Services - Provides mental healthcare for uninsured, underinsured and undocumented individuals and families.
  • Center for Community Health and Well-Being, Inc. - Provides direct primary and prenatal/post partum care to low income women.
  • Community Recovery Resources (Grass Valley) - provide individual and group counseling to chemically dependent individuals and their family members, primarily focusing on at risk adolescents and families.
  • Community Resource Project, Inc. - Provides prenatal care to low income women who are uninsured, underinsured or undocumented immigrants.
  • Domestic Violence and Assault Coalition (Grass Valley) - Offers educational presentations and counseling groups to pre-teens and teens to address issues and attitudes that can lead to partnership violence and sexual assault.
  • Falls Prevention Coalition of Nevada County (Grass Valley) - Provides education for physicians and their staff on risk factors correlated with falls and strategies for falls prevention, including assessment tools that will be incorporated into their practices. 
  • Food Bank of Yolo County (Woodland) - Seed to Table program will provide nutrition education and practical application featuring a community garden and a demonstration kitchen.
  • Head Trauma Support Project - Provides psychological support and therapeutic/recreational outlets for people with head injuries and caregivers
  • Homestead Supportive Housing Corporation (Davis) - Provides support for people with mental illness through direct services, case management for primary healthcare and support services referrals.
  • Hospice of the Foothills (Grass Valley) - Increase access to essential healthcare services and provide care coordination services, referral and transportation to those with a life-limiting illness who are underserved.
  • Midtown Medical Center - Provides a Russian speaking translator to improve quality of care delivered to underserved Slavic population.
  • Mutual Assistance Network - provides residents of Del Paso Heights area access to family wellness and nutrition programs at the Johnston Community Center.
  • My Sister's House - Provides direct mental health care services to children of victims of domestic violence.
  • Partners in Care - Establish a "Care Coordination Program" to evaluate individuals impacted by a reduction or loss of government and community services and fill the gap by providing case management, referrals and triage for those diagnosed with a life threatening illness and no support system.
  • Powerhouse Ministries, Inc. - Health Links program will increase access to healthcare for homeless and low-income individuals by providing referral and transportation services to primary care providers who accept Medi-CAL and also support of a Transitional Life Center that offers the homeless shelter and support services.
  • River Oak Center for Children - provides support to enhance the organization's home visitation program, and other family support programs.
  • Sacramento District Dental Foundation - Provides dental screening, treatment and education to uninsured and underinsured children ages one to 18.
  • Sierra Family Medical Clinic, Inc. (Nevada City) - Provide preventative dental health services to indigent, low-income, homeless, uninsured and underinsured as well as undocumented populations, which has been severely limited due to funding cuts.
  • Sierra Services for the Blind (Nevada City) - Develop program to provide increased services to the low income elderly who are legally blind and  face greater health issues due to age and mental capacity.
  • Society for the Blind - Transportation with specially trained drives to provide door to door free transportation to visually impaired people.
  • St. Patrick's Home for Children - Provides on-site therapists for its residential treatment center serving emotionally disturbed young women ages 11 to 18.
  • St. Vincent de Paul Society - Provides placement services for homeless families into transitional and permanent housing.
  • Stanford Settlement Neighborhood Center - Increase access to primary health and nutrition services, and wellness activities for low income elderly.
  • The Effort - provides direct mental health care to those with serious illnesses.
  • The Gathering Inn - provides drug testing and identification of persons who are in need of substance abuse treatment while living at the organization's shelter facility.
  • Volunteers of America - Support the Safe Ground initiative - a project to build a safe and healthy living environment for homeless individuals and families, while working to place them in permanent housing.
  • Women's Empowerment - Educate and empower homeless women with the skills and confidence necessary to get a job, maintain a healthy lifestyle and regain a  home for themselves and their children.
  • Yolo Family Service Agency (Woodland) - Low-income counseling services to subsidize the cost of counseling for parents and children who do not have private insurance and cannot afford counseling from private practitioners.
  • Yolo Wayfarer Center (Woodland) - Provides continuum of care through homeless shelter, substance abuse treatment, case management and referral, transportation.

CHW has distributed $5.3 million to 247 organizations in the Sacramento area and $34.6 million to deserving projects throughout the CHW system. Grant recipients are required to complete a detailed grant proposal outlining the purpose and scope of the project to be funded, as well as the population to be served, the project's outcomes, plans for its long-term operation and measurement of success. The projects must be in the area of healthcare, social services, education, economic development or holistic health.
