Mercy Home Health provides comprehensive care for patients who have recently been discharged from the hospital and homebound individuals who need short-term, skilled nursing or therapeutic care.
Enhancing Independence We work with patients to help them achieve their health objectives, whether that's full recovery after cardiac surgery or the ability to walk without aid after a hip replacement. In each case, our goal is to make patients as independent and self-sufficient as possible.
Individualized Care Plans Based on physician orders and an initial physical assessment, our Home Health team develops a plan of treatment for each patient, with clearly stated goals and desired outcomes. Our professionals perform whatever skilled services are needed, from intravenous therapies to wound care and physical, occupational and speech therapy. They closely monitor every patient's progress, enabling them to meet their health objectives as rapidly as possible.
Certified, Professional Care Teams Mercy Home Health professionals are all state-licensed, Medicare-certified and JCAHO-accredited. Our clinically skilled home care teams may include:
